1/10/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She indicated to QMHP-C that she was good. QMHP-C discussed with Ashiyah that this session was going to focus on her goals as mother reports sat times her behaviors will last up to 10 minutes in duration when she cannot have her way. 2. Behavior: Ashiyah actively participated in the session and needed some redirection at times as she was hostile with her mother by some of the things that she was discussing as it relates to her behaviors being presented while behaviors can last up to 10 minutes in duration. 1.Intervention: QMHP-C began session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C provided Ms. Hodge with a copy of worksheets that would be provided and updated Ms. Hodge on what has been covered thus far in sessions. QMHP-C engaged Ashiyah and Ms. Hodge in a conversation regarding a non-confrontational topic in order to help Ashiyah and Ms. Hodge develop a stronger relationship. 2. Intervention: QMHP-C observed Ashiyah and Ms. Hodge interacting in an appropriate fashion free from any sarcastic remarks or rude behaviors. QMHP-C allowed Ashiyah to openly discuss with QMHP-C and her mother her feelings as QMHP-C processed thoughts and feelings expressed. QMHP-C engaged Ashiyah in an activity about her family and relationships with family members in the home to include which members she has the most conflict with, who she respects the most and closest to among others. 1. Response: Ashiyah and her mother appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ms. Hodge informed QMHP-C that things have been going well in the home with Ashiyah and that there have not been any anger outbursts. Ms. Hodge actively listened as she was updated on session’s content and what Ashiyah has learned thus far in sessions. Ms. Hodge and Ashiyah actively engaged in conversion often laughing at each other’s comments both exhibited appropriate behaviors throughout conversation. 2. Response: Ashiyah engaged in activity noting the different relationships she has with family members such as admiring her mother the most out of her family members because of the way she takes care of her needs and wants.
